Unactioned CHA muting and death tasks

Hi design community,
Looking at this CHP persona and the empathy map above, how might we redesign the flow for the CHP to get a better user experience and, at the same time, make the CHP supervisor aware of the changes?

Context:
The Current flow;
CHP submits the mute person or mute household report then the CHP supervisor (CHA) gets mute person/household approval. Task window is a week. Seems like these CHA tasks disappear without being actioned.

More Context,
CHPs are in charge of at least 100 HH
A HH has an average of 5 people
CHAs supervise at least 10 CHPs

The Problem(s):
CHPs are reporting that they are still seeing people/households they muted.
Similarly they are still seeing the people they reported as dead because the CHAs have’nt completed their tasks in a timely
Frequent system downtimes CHP supervisors not being able to log into their applications frequently because of other technical challenges.
At the end the CHP would like accurate population and household information on their application
