Program’s Goals and Target Population:
The primary goal of gandaki province NCD (Non-Communicable Diseases) program is to provide timely health information and guidance to individuals who have high lab test results indicating potential risks of NCDs. This will be achieved through an SMS-based alert system that notifies individuals at risk, helping them take proactive steps towards managing their health. Our target population includes individuals who have undergone lab tests and show high results related to NCD markers, such as high blood pressure, cholesterol, or blood glucose levels. -
Utilization of the CHT (Community Health Toolkit):
Health directorate plan to utilize the Community Health Toolkit (CHT) to manage and track the individuals at risk, as it will allow for seamless integration of lab results and follow-up actions through SMS. The CHT will be instrumental in ensuring that high-risk individuals are continuously monitored, and the system can generate alerts and health guidance automatically based on predefined thresholds of lab results.

We foresee several technical challenges in implementing this system, including:
