Where / how do we upload images that can be used in targets for example as icons?

In the Configuration pages, go to Icons: https://demo.dev.medicmobile.org/medic/_design/me…
Note that the icons are not resized automatically, so it is good to resize them manually before uploading. I have found that having them 22px tall works well.
