After installing TLS certificate, Couldn't add the facility

After adding the TLS , not being able to add the facility.
What do i missed out here ?

Could you please paste the error(s) logged on the browser console. Error loading form points to an error when parsing the form structure.

@sanjay just wanted to check if the forms were uploaded using the following

medic-conf --local convert-contact-forms upload-contact-form
