The title states it:
We would like to display a small table inside the form and wonder if that is possible? For instance as an html table? I would rather not use a hard-coded image file, as this is bad practice.

Technically, you should be able to include a raw HTML table right in the label for your note field.
Styling it might be a bit tricky, but it should try to render whatever you put in there.
